Communicate immediately. If you feel unwell during dinner, tell the companion. She will understand. If you need to end the evening early due to illness, pay for the time spent and apologise. A professional companion will be sympathetic and may even help you manage the situation. If you feel unwell before the booking, cancel with as much notice as possible. Your health comes first and any reasonable companion understands that.
